Job Description: Registered Nurse (Home Care)

Responsibilities:

• Provide skilled nursing care to patients in their home setting
• Assess patient's condition and develop a plan of care
• Administer medications and treatments as prescribed by the physician
• Educate patients and families on health maintenance and disease prevention
• Coordinate care with other healthcare providers and community resources
• Document patient care and progress notes
• Maintain accurate and up-to-date patient records

Skill Sets:

• Current RN license in the state of practice
• Experience in home health or a similar setting preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and prioritize tasks
• Knowledge of electronic health records and computer proficiency
• Compassionate and caring attitude towards patients
