Responsibilities:

• Assess patients' dietary needs and create personalized nutrition plans
• Educate patients on healthy eating habits and lifestyle changes
• Monitor and track patients' progress and adjust nutrition plans as needed
• Counsel patients on special dietary needs such as diabetes, food allergies, and weight management
• Work with healthcare teams to provide comprehensive care for patients
• Lead group nutrition education sessions
